Declaration and Undertaking on Data Accuracy

I, the applicant, in my capacity as the owner of the facility or its legally authorised representative, declare that: 1. All data, information, documents, photos, videos and geographic coordinates that I entered or attached to this request are true, accurate, complete and up to date, and match the facility's official records. 2. I bear full legal responsibility for the accuracy of what I have submitted, and for any damage, fine, rejection or cancellation resulting from incorrect, misleading or incomplete data; the Company bears no liability in that respect. 3. I hold the right or sufficient authority to upload everything I attached, and the attached content does not infringe the rights or privacy of others and does not include images or data of individuals without their knowledge or consent where required. 4. I agree that the Company may verify the data with competent authorities and official sources, and may reject the request, suspend its processing or cancel its consequences whenever any data is found to be inaccurate. 5. I undertake to notify the Company immediately of any change to the submitted data.

Consent to Collection and Processing of Data and Attachments

I expressly consent to the Company collecting and processing the data I provide in this request, including: identity and contact details (name, mobile, email), facility details (unified number, commercial registration, activity, national address), geographic location, attached photos, videos and documents, and the connection and device data needed to secure the request (IP address and browser or device type). This is for the purpose of reviewing the request, issuing the requested certificate or contract and submitting it to the competent authorities where required, contacting me about it, and keeping the statutory evidence record. The data is retained for as long as needed for these purposes or as required by regulations, and is shared only with competent government authorities, service providers bound by confidentiality, or where legally required. I have the right to access my data, correct it and request its deletion in accordance with the Personal Data Protection Law, by contacting the Company.
